It puts physician bios, accepted insurance, and location/hours above the fold (the three things a prospective patient checks before scheduling), then offers two intake paths: New Patient Intake (longer form, captures contact and reason for visit) and Request Appointment for established patients. The forms capture scheduling and contact data only — not protected health information — so the practice keeps its HIPAA workflow inside the EHR, not bolted onto a marketing tool that wasn't designed for it.
How an intake becomes a patient
Each New Patient Intake creates a Contact in your CRM Cubby tagged with the requested provider, insurance, and visit type. Front-desk staff move new patients through a pipeline: Intake Received → Insurance Verified → First Appointment Scheduled → First Appointment Completed → Established Patient. Established patients submitting Request Appointment skip the intake stage and route directly to the right physician's calendar. For the actual visit notes, lab results, and protected health information, your EHR (Epic, Athenahealth, eClinicalWorks, Practice Fusion, DrChrono) remains the system of record — Instaform handles the public-facing scheduling and intake; the EHR handles the medicine. A webhook syncs Contact data into the EHR as a new patient record once intake is complete. Pair the page with a custom domain (yourpractice.com or new-patient.yourpractice.com) so the URL on referring physician handouts and insurance directories matches your practice.
