Who this template is for

An internship application is not a shorter version of a job application — it's a different document with different stakes. Internship hiring runs on a calendar most full-time pipelines don't share: summer cohorts open recruitment 6-9 months ahead, the candidate pool is dominated by students you can't reach through LinkedIn the same way, and the form itself has to capture a specific set of details that a job application doesn't — current school and program, expected graduation date, availability windows tied to academic calendars, and the work-authorization specifics that determine whether an offer can even be made (F-1 students on OPT/CPT in the US, study-visa holders in the UK and EU, students operating under their school's official internship agreement in Brazil and Spain). This template is designed for that program-style intake. It captures the right academic context up front, makes the work-authorization questions explicit (without becoming an EEO landmine), and routes submissions into a cohort-aware pipeline — the kind you need when you're triaging 800 applications for 30 spots in a structured summer program.

Cohort intake, structured screening, and clean ATS handoff

Submissions flow into your ATS as a tagged "intern" record — Greenhouse, Lever, Workday Recruiting, Ashby, BambooHR, or via webhook into spreadsheet-based pipelines if you're a smaller program. The school + program fields auto-feed into your university-relations dashboard if you're tracking which programs you draw from. The availability window flags candidates outside your target cohort (summer-only vs. semester vs. year-round) for separate routing. The work-authorization questions — "Are you legally authorized to work in [country] for the duration of this internship?" and the country-specific follow-ups — route candidates with sponsorship needs to a clear yes/no decision before phone-screen time is invested. Portfolio and project links route to the technical reviewer queue (engineering and design programs) ahead of recruiter screen, because for early-career candidates the work product is more predictive than the resume. The auto-acknowledgement email sets expectations on timing — most structured internship programs commit to a 2-4 week first-response SLA, which the email should communicate explicitly so candidates aren't black-boxed.

What's included

Every field below is here because internship hiring needs context a generic job-application form misses: academic calendar, school relationship, work-authorization specifics, and the portfolio/project evidence that often matters more than a half-page resume for an early-career candidate. Customize per program and per region.

Designed for the programs that actually run structured internships

  • Tech engineering and product internships

Software, ML, data, design, product, security — internship pipelines for tech roles run on a tight calendar (early-fall recruiting for summer cohort), high volume (often 100x oversubscribed at named companies), and project-portfolio-driven screening. Add a GitHub/portfolio field as required, a free-response "describe a project you're proud of and what you'd do differently" question (signal-rich, hard to fake), and a coding-assessment link delivered post-submission. For OPT/CPT eligibility, surface the F-1 status field early so the recruiter team knows whether to route through immigration counsel.

  • Finance, banking, and consulting internships

Structured summer programs (IB summer analyst, MBB consulting summer associate, asset management) recruit 12+ months ahead and run a specific case-study-driven screening process. The application here needs to capture GPA (industry-standard), target program (BS/BA, MBA), school's CFA Affiliated Program status if relevant, and prior finance experience. Make the cover letter or "why our firm specifically" field mandatory — boilerplate cover letters are the single fastest screening filter in finance recruiting.

  • Marketing, comms, and agency internships

Often unpaid or partially paid, often coursework-credit-bearing, often high-volume. The application should surface the candidate's coursework-credit status (FLSA primary-beneficiary defensibility), portfolio of writing/campaign work, and social-media presence (this is one of the few internship contexts where the candidate's own social account is legitimately relevant signal). Be explicit about whether the internship is paid, unpaid-for-credit, or stipended, in the form description and the auto-acknowledgement — candidates who applied expecting paid roles drop off mid-process otherwise.

  • Nonprofit and mission-driven organizations

Mission-driven applicants self-select hard for nonprofit internships, but the program still needs to filter for fit and capacity. The "why our mission" field reveals more than any other question — boilerplate answers correlate with high attrition mid-summer. For nonprofits using AmeriCorps VISTA, university service-learning, or formal fellowship programs (Coro, Public Policy and International Affairs), capture the program type explicitly so the offer letter and stipend structure routes correctly. Many nonprofits also offer credit-only internships with attached university coordinators — capture the coordinator name and email up front.

  • Creative, media, and entertainment internships

Publishing, fashion, music, film, gaming, agency creative — heavy on portfolios and lighter on traditional resumes. Make the portfolio field required and the resume field optional or secondary. For entertainment internships, the union and credit considerations matter (SAG-AFTRA on the talent side, IATSE on the production side, WGA on the writing side); even when the internship itself is non-union, the candidate may need to disclose existing union memberships. For fashion, magazine, and creative-agency programs in NYC/LA/London/Paris/Milan, the implicit cost-of-living gatekeeping (unpaid + expensive city) is real — be explicit about pay and housing assistance in the form description.

  • Government, public service, and policy internships

US Pathways Programs, congressional internships, federal agency programs, presidential management fellows, state and local equivalents — each has very specific eligibility criteria (citizenship, school enrollment status, GPA thresholds, sometimes specific academic programs). The application here needs structured eligibility-confirmation questions ("Are you a US citizen or eligible national? Are you currently enrolled at an accredited institution? What is your current GPA?") with required documentation upload (transcript, enrollment verification). For state and local government, similar structures apply with regional variations.

Fit the form to your program's structure, not the other way around

Add cohort-specific fields per program (summer technical, semester product, year-round operations) and use conditional logic so each candidate only sees the relevant questions. Make the work-authorization question wording match your jurisdiction's framework — in the US, "Are you legally authorized to work in the US? Will you require sponsorship now or in the future?" is the standard pair, and the second question is what gates downstream EEO compliance. Add a specific OPT/CPT status field for international student programs in the US — "F-1 visa on CPT (Curricular Practical Training) with my school's authorization" vs. "F-1 on OPT (Optional Practical Training) post-graduation" vs. "requires future sponsorship" — because each of those routes through a different recruitment process. For programs accepting candidates from multiple regions, add language preferences and locale-specific work-authorization questions in a single form using conditional branching on country. For unpaid or stipended internships, make the FLSA primary-beneficiary acknowledgement field explicit and date-stamped — that documentation matters if a candidate later claims employee status.

Internship application form FAQ

Three structural differences. First, the academic calendar — internships are bound to school terms, and the application has to capture current school, program, expected graduation, and availability windows that map to academic semesters. Second, the work-authorization framework — F-1 students on CPT/OPT in the US, study-visa holders in the EU/UK, students under their university's internship convenio in Spain, students with a TCE under Lei do Estágio in Brazil. Each has specific requirements that a generic "are you authorized to work" question doesn't capture. Third, the screening evidence — for early-career candidates, portfolios, coursework, and project links are usually more predictive than the resume, so the form should weight those fields up. A generic job application asking for 5+ years of experience and references doesn't work for internship intake.
Industry-dependent. Finance, banking, consulting, and accounting still treat GPA as a primary screening signal — collect it required, with the option to attach an unofficial transcript. Tech (especially engineering, design, ML) increasingly weights portfolio/GitHub/project work above GPA; collect GPA as optional, portfolio as required. Marketing, communications, and creative fields are portfolio-dominant — make the portfolio link required and the GPA optional. Nonprofit and public service vary by program; check the host program's published requirements (Pathways, AmeriCorps, congressional offices all publish their criteria). When in doubt, make GPA optional and portfolio/project links the required differentiator — this also reduces the bias that purely GPA-driven screening tends to introduce.
Each jurisdiction has its own framework, and the application form is your first-line compliance artifact. United States: the DOL primary-beneficiary 7-factor test applies — the intern must be the primary beneficiary, not the employer. Build a checklist into the application acknowledgement: course credit alignment, mentorship structure, no displacement of paid workers, no guaranteed employment. Brazil: Lei do Estágio 11.788/2008 requires a Termo de Compromisso de Estágio (TCE) with an institution of education and an agente de integração (CIEE, IEL, Núcleo Brasileiro de Estágios) — collect the school and the agente up front. Spain: prácticas curriculares (with university convenio) vs. extracurriculares (RD 1493/2011, max 50% of workday, beca mínima typically ~600€) — capture which type and the school convenio reference. UK: National Minimum Wage law applies once the role involves "set hours and tasks" — unpaid internships in for-profit roles are largely no longer compliant; structure as paid or as work-experience (under 1 month, observational). When in doubt: consult employment counsel.
Yes — every submission can webhook into your ATS as a tagged intern candidate. Greenhouse and Lever have direct webhook intake; Workday Recruiting typically routes via a structured CSV import or middleware; Ashby has native webhook support. For Brazilian programs, Gupy (the dominant Brazilian ATS for early-career hiring) and Solides accept webhook integration. The cohort tag in the webhook payload (e.g., "summer-2026-engineering") lets the ATS group submissions by program for batch screening. If your university-relations team uses a separate platform (Handshake, Symplicity), Instaform can also feed that pipeline — Handshake accepts external application webhooks for employer programs.
Structure the question explicitly and conditionally. United States: "Are you currently enrolled at a US institution of higher education? If yes: undergraduate / master's / PhD; international student on F-1 with CPT authorization / international student on F-1 considering OPT / US citizen or permanent resident / other." The CPT path requires school authorization at the time of offer, so flag those candidates early. Brazil: "Estás matriculado em curso superior, técnico ou EJA? Qual instituição? Você tem TCE assinado, ou precisa que iniciemos o processo com um agente de integração?" — many Brazilian companies handle the TCE through CIEE or IEL on the candidate's behalf, but the form needs to capture the candidate's current status. Spain: "¿Estás matriculado/a actualmente en una universidad española? ¿Tu universidad tiene convenio de prácticas con nuestra empresa, o necesitamos iniciar uno?" — the convenio is the legal vehicle for the internship in Spain, so this question gates downstream paperwork.
