Product review form — capture verified, compliant reviews that drive conversion
A review-capture form with star rating, title, body, photo/video upload, verified-purchase tag, and the incentivized-review disclosure that the FTC's fake-review rule and the EU Omnibus Directive now require — built to feed your Trustpilot, Yotpo, Bazaarvoice, Okendo, or Loox review widget without compliance gaps.

Who this template is for
Product reviews are simultaneously the highest-converting and most-regulated content on a commerce site in 2026. The conversion math is unambiguous — reviews drive 30-40% of e-commerce purchase decisions across categories, products with photo or video reviews convert 2-3x better than text-only reviewed products at the same star rating, and a single missing star (a 4.4 vs. a 4.5) measurably depresses sell-through at scale. The regulatory math has caught up. The FTC's final rule on fake reviews and testimonials (effective October 2024) prohibits buying, selling, soliciting from employees without disclosure, or suppressing genuine negative reviews, with penalties up to $50,000+ per violation. The EU's Omnibus Directive requires verified-purchase distinction and explicit disclosure when reviews are incentivized. Brazil's Procon and Senacon have been enforcing similar standards under the Código de Defesa do Consumidor, with Reclame Aqui as the parallel public-shaming mechanism that runs alongside any private review system. The review form most brands ship — generic star + text, no photo prompt, no verified-purchase tag, no disclosure field — is no longer compliant in any of the three major markets. This template gives you the right structure: star rating, headline, body, optional photo and video upload (the conversion multiplier), verified-purchase indicator that auto-attaches from your order data, and the incentivized-review disclosure checkbox that satisfies FTC + EU + Brazilian requirements simultaneously. The reviews flow into your review widget of choice (Trustpilot, Yotpo, Bazaarvoice, Okendo, Loox, Reviews.io, Stamped, Judge.me; for Brazilian e-commerce, Yampi / Nuvemshop / Loja Integrada native modules; for B2B SaaS, G2 / Capterra / TrustRadius) without you having to bolt on a separate compliance layer.
From post-purchase prompt to verified-purchase review in 90 seconds
The review prompt fires after a purchase event with appropriate timing — typically 7-14 days post-delivery for physical products, 3-7 days post-activation for digital, immediately after the experience for services. The trigger can be email, SMS, in-app modal, or QR code on packaging. The form loads with the product context already populated (which order, which SKU, which variant — captured from the order ID in the link). The reviewer sees the star rating (1-5, with hover labels — "poor / fair / good / very good / excellent"), the headline field (the search-discoverable summary), the body field (the detailed review with appropriate character minimum to filter low-effort submissions), the photo/video upload (optional but heavily encouraged — visual reviews are the single highest-conversion signal), the would-recommend toggle (separate from star rating because the two correlate but aren't identical), and the optional structured fields per category (size/fit for apparel, durability/quality for hardware, ease-of-use for SaaS). The verified-purchase indicator auto-attaches from the order data (no user action required). The incentivized-review disclosure is a single checkbox that surfaces only if the brand has incentivized this specific review (gift card, free product, discount in exchange for review) — when checked, the review publishes with a visible "incentivized" badge as the FTC and EU rules require. On submission, the review flows to the review platform (Trustpilot API, Yotpo, Bazaarvoice, Okendo, Loox, Reviews.io, Stamped, Judge.me; Yampi / Nuvemshop / Loja Integrada for Brazilian e-commerce; G2 / Capterra / TrustRadius for B2B SaaS), into the moderation queue if your policy is pre-moderation, and into your CRM as a customer-engagement event.
What's included
Each field below is here either to drive the conversion-side of reviews (photo upload, headline, would-recommend) or to satisfy the compliance-side (verified-purchase, incentivized disclosure, age verification when applicable). Skip what doesn't apply to your specific category, but resist removing the verified-purchase indicator and the disclosure field — those are the regulatory pillars in 2025-2026.
Built for the categories where reviews directly drive conversion and trust
E-commerce and DTC brands
The primary use case. DTC apparel (Allbirds, Everlane, Outdoor Voices, Cuyana, Reformation; Brazilian DTC like Insider, Amaro, Reserva, Animale, Farm; Spanish DTC like Hawkers, Pompeii Brand, Sepiia), DTC beauty (Glossier, Drunk Elephant, The Ordinary, Rare Beauty; Brazilian beauty Sallve, Simple Organic, B.O.B; Spanish Druni-DTC, Freshly Cosmetics), DTC home (Brooklinen, Parachute, Burrow, Avocado; Brazilian Tok&Stok DTC line, Etna DTC, Mobly DTC), DTC supplements (Athletic Greens, Ritual, Care/of; Brazilian Vitamina Mais, Puravida, Bioage). All of these run on review-driven conversion — reviews displayed on product pages, in paid ad creative, in email-marketing, and in landing pages. The verified-purchase tag and photo/video upload are the two conversion levers; the FTC fake-review rule and EU Omnibus Directive are the compliance floor. For Brazilian DTC specifically, the Reclame Aqui parallel reputation matters as much as the on-site star reviews — Brazilian consumers cross-check both before purchasing, and a strong star rating with poor Reclame Aqui score is a trust-broken combination.
Multi-seller marketplaces
Different dynamic because the marketplace owns the review system and individual sellers compete on review profile. Amazon, eBay, Etsy, Walmart Marketplace, Target Plus, AliExpress, Temu, SHEIN; MercadoLivre (Brazilian dominant), Magalu Marketplace, Americanas Marketplace, Shopee Brasil, Amazon Brasil. The marketplace's review policies are the rules of engagement, but individual sellers can run additional review-collection through their own packaging inserts, post-purchase emails, and review platforms (Yotpo, Bazaarvoice, etc.) — these feed both the marketplace review system (via API) and the seller's own off-marketplace surfaces. For Brazilian marketplaces, the rule of thumb is that Reclame Aqui score weighs as much as the marketplace's own review score in consumer trust, and Brazilian sellers who actively manage both signals systematically outperform those who only optimize one. Marketplace fake-review enforcement has been heavy in 2024-2026 — Amazon has banned tens of thousands of sellers and millions of fake reviews; MercadoLivre and Magalu have followed similar enforcement patterns.
Service businesses (Yelp, Google, TripAdvisor, restaurant, salon, repair)
Reviews here are particularly high-stakes because the service has already been delivered and the review reflects accumulated experience, not a single transaction. Restaurants (Yelp, Google, TripAdvisor, OpenTable; Brazilian TripAdvisor, Folha de S.Paulo restaurant guide, ifood reviews for delivery), hair and beauty salons (Yelp, Google; Brazilian Booksy reviews, Belezix reviews), home services (Angi, HomeAdvisor, Thumbtack, TaskRabbit; Brazilian GetNinjas reviews), auto repair, dental and medical practices. The verified-customer tag for service businesses typically comes from booking system integration (the review form is sent only to customers who completed a booking, not walk-ins or anonymous reviewers). The FTC fake-review rule applies as strongly here as in e-commerce, and the EU enforcement under the Omnibus Directive has been notably active against service-business review manipulation. For Brazilian service businesses, Reclame Aqui is the dominant complaint signal alongside Google reviews.
B2B SaaS review platforms (G2, Capterra, TrustRadius)
Different audience and different review-collection dynamic. B2B SaaS reviews on G2, Capterra, TrustRadius, Software Advice, GetApp, PeerSpot drive sales-cycle decisions for enterprise buyers, who systematically read 5-15 reviews before shortlisting a vendor. The review form for B2B SaaS is longer than consumer (typically 15-25 questions vs. 5-10 for consumer) because the value of the review to a future buyer scales with detail. Standard fields: company size, industry, role of reviewer, what problem the product solves, what they like best, what they like least, what alternatives they considered, would-recommend, pricing fairness, support quality, integration breadth. G2 and Capterra both have their own verified-reviewer programs (G2's authentication via LinkedIn or business email; Capterra's verification via screenshot). Incentivized reviews on B2B SaaS platforms are typically gift-card based and require disclosure under FTC rules; G2 and Capterra surface the incentivized tag prominently. For Brazilian B2B SaaS (RD Station, Pipefy, Conta Azul, Movidesk, etc.), the same platforms apply alongside B2B Stack as a Brazilian-specific review site.
Hospitality (Booking, Airbnb, Hotels.com, TripAdvisor)
Reviews drive a measurable percentage of hotel and short-term-rental bookings (industry studies put it at 50-70% for first-time travelers to a destination). The platform owns the review system here — Booking, Airbnb, Hotels.com, Expedia, Hotwire, Trip.com, TripAdvisor — and the property has limited ability to collect reviews outside that platform. The review timing matters: Booking and Airbnb both prompt reviews automatically post-checkout, with limited windows (Airbnb's 14-day window is the canonical example), and the rule of thumb is that hosts and properties who respond promptly to reviews (especially negative ones) outperform those who don't. For Brazilian hospitality, Booking and Airbnb dominate; Hoteis.com and Decolar are also active. For Spanish hospitality, Booking, Hotels.com, and the regional Iberian-specific platforms (Atrapalo Hoteles, eDreams Hoteles). For US hospitality, Yelp and Google Reviews matter alongside Booking and TripAdvisor — and the FTC fake-review rule has been actively enforced against hospitality businesses that paid for fake positive reviews.
App reviews (App Store, Play Store)
Covered partially by the mobile-app-feedback template's sentiment-gating, but worth noting for completeness. App Store and Play Store reviews are platform-owned — the app cannot run its own review system in lieu of the platform's, but it can capture intent (via sentiment-gated routing) before redirecting to the platform's native review UI. The FTC fake-review rule applies to app store reviews as much as to e-commerce reviews, and Apple and Google both run their own fake-review enforcement that has removed millions of reviews and banned tens of thousands of accounts. For Brazilian app store reviews, the dominant local apps (Nubank, iFood, MercadoLivre, PicPay, Magazine Luiza) have outsized App Store and Play Store presence and their review profiles directly affect install conversion in the local market.
Configure the review form to your category and your regulatory exposure
Start with the timing. Post-purchase review prompts should fire 7-14 days post-delivery for physical products (long enough that the customer has actually used the product, short enough that the experience is fresh), 3-7 days post-activation for digital products, immediately after the service for services, and 1-7 days post-checkout for hospitality. Avoid prompting before the customer has had a real experience — premature prompts generate lower-quality reviews and (worse) get flagged by review platforms' fake-review detection systems. Add the verified-purchase tag via integration with your order data — the review form auto-receives the order ID from the prompt link, confirms the customer's purchase, and attaches the verified-purchase indicator without the customer doing anything. Add the incentivized-review disclosure checkbox that surfaces only when the brand has incentivized this specific review — the checkbox should default to checked when the prompt came from an incentivized campaign (gift card, free product, discount) and stay hidden otherwise. Encourage photo and video upload prominently — visual reviews convert downstream purchase 2-3x better than text-only, and most customers will upload if asked. Add structured per-category fields (size/fit/quality for apparel; ease-of-use/curriculum-depth for courses; check-in/cleanliness/value for hospitality) because these aggregate into useful per-product analytics that the star rating alone can't reveal. For moderation, decide between pre-moderation (every review screened before publication, slower but compliant-safer) or post-moderation (reviews publish immediately with takedown for violations, faster but requires active monitoring). FTC fake-review rule explicitly prohibits suppression of genuine negative reviews — moderation is for fraud detection, not negative-review suppression. For Brazilian e-commerce, integrate the Reclame Aqui pre-resolution workflow as a parallel signal capture — capture dissatisfaction in the review form when a 1-star is submitted and offer a service-recovery path before the review publishes publicly.
