Who this template is for
B2B sales teams, executives and chiefs of staff, customer success teams running QBRs, consultants intaking client meetings, internal cross-functional coordinators, and exec assistants triaging inbound requests use this template to replace the "let's find a time" email thread that wastes a week before the meeting actually happens. It captures topic, meeting type, duration, preferred dates and times, agenda items, and required attendees — the seven things that determine whether a meeting will be substantive or another 30 minutes nobody remembers. Each request creates a Contact in your CRM Cubby tagged by topic and meeting type, so the routing to the right calendar (sales meetings to AEs, support to CS, partnership to BD) is automatic. Pre-meeting agenda capture lifts meeting quality 40-60% — people show up prepared instead of asking "what are we here for?" in the first five minutes.
How a request becomes a meeting worth having
Each meeting request creates a Contact in your CRM Cubby tagged with the topic, meeting type (sales / support / partnership / internal), and required attendees. Route the request to the right calendar: sales to the AE who owns the account, support to the CS lead, partnership to BD. The agenda items attach as a Note on the Contact so the attendee can read them before the meeting — not in the first 10 minutes of it. For sales-led meetings, the pipeline runs: Requested → Scheduled → Held → Qualified / Not Qualified → Proposal (if qualified). For internal meetings, the same Contact tracks attendance, action items, and the outcome. Pair the form with Calendly or Cal.com embed on the confirmation page — the meeting-request form captures the context, and the calendar embed handles the actual slot picking. Poorly-prepped meetings waste 4-8 person-hours of corporate time per meeting; this fixes that at the intake stage.

Teams using this template
B2B sales teams (AEs and SDRs)
Pre-discovery-call intake — captures use case, stakeholders, and budget context before the call, so the call is qualifying, not introducing.
Customer success teams (QBR / EBR scheduling)
Quarterly and executive business reviews need agenda alignment ahead of time — the form captures the customer's discussion topics so CS arrives with the right artifacts.
Executives and chiefs of staff
Exec assistants triage inbound meeting requests by topic and urgency; structured intake replaces the "can you sync with my boss?" email thread.
Consultants intaking client meetings
Hourly billing makes prep mandatory — agenda capture means the consultant can review materials before the meter starts running.
Internal cross-functional coordinators
Required-attendees field prevents the "wait, did you invite finance?" delay that kills a third of cross-team meetings.
L&D and training coordinators
Training session requests captured with skill area, attendee count, and required prerequisites — feeds the curriculum design before the session is scheduled.
Customization notes
Most teams add a topic-routing dropdown (Sales / Support / Partnership / Internal / Other) that drives which calendar the request lands on, a required-attendees-vs-optional-attendees split (helps the host know who's load-bearing for the decision), an upload field for pre-meeting materials (decks, contracts, exhibits — the pre-read is what turns a 30-min meeting into a 30-min decision), and a recording-consent checkbox for sales calls or external meetings (increasingly legally required, especially with two-party-consent states and EU GDPR). For exec assistants, add a priority field (Routine / This Week / Urgent) and a calendar-specific routing rule.
