Who this template is for

Country clubs, professional associations, boutique gyms, coworking spaces, alumni networks, private online communities, masterminds, and faith communities use this template when membership is curated, not instant. It captures everything a review committee actually needs to decide — full contact, date of birth, address, the membership tier requested, the reason for applying, and (the key field) who referred them. Each application creates a Contact in your CRM Cubby in an "Application Received" pipeline stage, so the review team has a clean queue every Monday morning — not a stack of forwarded emails. Approved applicants move to "Member"; declined applicants get a polite, templated response. The whole audit trail lives on the Contact record.

How the application becomes a member

Each application creates a Contact in your CRM Cubby in the "Application Received" stage of your membership pipeline. The referred-by field becomes a tag, so you can filter applicants referred by existing members for priority review (or for the discount the referring member earns). Assign each application to a reviewer with a one-week due date; the reviewer reads the "why join" essay, checks the referrer, and moves the Contact to "Approved," "Waitlist," or "Declined." Approved members move into your dues-payment workflow (Stripe, PayPal, or your payment processor); declined applicants get a templated reply that keeps the door open for re-application later. The rules-acceptance checkbox creates a timestamped audit record tied to whichever version of your bylaws was in effect that day.

What's included

The fields a review committee actually reads — identity, referrer, motivation, rules acceptance. Rename, reorder, or remove any field in the editor.

Organizations using this template

  • Private clubs and country clubs

Curated membership with a member-referral requirement; review committee runs the approval workflow off the same pipeline.

  • Professional associations and certifications

Industry groups, certification bodies, and chambers — capture credentials and verification fields alongside the motivation essay.

  • Boutique gyms and fitness communities

Niche or capped-roster gyms that interview before granting access; pair the form with a waiver-acknowledgment checkbox.

  • Coworking spaces and innovation hubs

Curated coworking communities (founders, designers, writers) where fit matters more than filling seats — review the motivation essay before approving.

  • Online communities and masterminds

Paid Slack/Discord/Circle communities, masterminds, peer groups — vet applicants by referrer and goals before granting access.

  • Alumni associations and faith communities

Member-based organizations where identity verification, demographic data, and rules acceptance form the standard intake.

Customization notes

Most organizations add an interview-availability field (calendar slots) for memberships that include a vetting call, a portfolio or CV upload for professional associations and creative communities, a multi-select for membership tier (Basic / Premium / Lifetime) so each tier maps to its own onboarding sequence, and a references field (two existing members' names) for the most exclusive clubs. Pair the form with your payment processor — once a Contact moves to "Approved," auto-send the dues invoice or subscription checkout link.

Yes. Every Instaform template is free on every plan, including the Free tier. No per-application fees, no premium tier.
Each submission creates a Contact in your CRM Cubby in an "Application Received" pipeline stage. Assign reviewers, set due dates, and move applicants through Approved / Waitlist / Declined stages as the review committee decides. Approved members move into your billing workflow; declined applicants get a templated polite reply. The whole audit trail (who reviewed, when, what they wrote) lives on the Contact.
A registration form grants instant access — the user fills it out and they're a member. An application form starts a review process — the form captures the data, but a real human (or committee) decides yes/no. This template includes the fields a reviewer actually reads (referrer, motivation essay, rules agreement) and is designed to live inside a CRM pipeline with stages, not a one-click signup.
Yes. Add a references field for existing-member names, a calendar field for interview availability, or a background-check consent checkbox in the editor. For interview-required memberships, set the form to auto-create a calendar event when submitted, or link to Calendly / Cal.com in the confirmation screen so applicants book their slot before the committee meets.
Yes — English, Spanish, and Brazilian Portuguese. Your applicants see the form in their preferred language; your CRM stores every application with the original locale attached, so the welcome packet goes out in the right tongue once they're approved.
