Healthcare forms carry obligations most other forms do not, and the design has to start from that rather than bolt it on. Collect the minimum clinically necessary information, be explicit about what it will be used for, and keep anything sensitive out of email entirely.

Split intake into two stages. Scheduling needs a name, contact details and a reason for the visit; clinical history belongs in a separate form sent after the appointment is confirmed, when the patient is committed and the context is clear. Combining them produces abandoned bookings and incomplete histories at once.

Write consent in plain language and keep it separate from any marketing permission. A patient agreeing to be contacted about their appointment has not agreed to a newsletter, and conflating the two is both a compliance risk and a trust problem.

Instaform keeps submissions inside the application rather than forwarding them by email, with each patient as a Contact carrying their appointment history. Practically, that means front-desk staff can see prior visits and notes without opening a mail client, and sensitive detail is not sitting in several inboxes indefinitely.

FAQ

What should a patient intake form ask?
Split it. Scheduling needs name, contact and reason for visit. Clinical history belongs in a second form after the appointment is confirmed — combining them costs you both bookings and complete histories.
How should consent be handled?
Plain language, and strictly separate from marketing permission. Agreeing to be contacted about an appointment is not agreeing to a newsletter.
Is it safe to email patient forms?
No. Submissions should stay inside the application rather than being forwarded, so sensitive detail is not sitting in multiple inboxes indefinitely.
Can I see a patient history at the front desk?
Each patient is a Contact carrying appointment history and notes, so prior visits are visible without opening a mail client.
