Who this template is for
Salons, clinics, studios, repair shops and consultants use this template when appointments run on set times. A client picks a service, a date and a time, leaves their contact details and says whether this is their first visit, so you know who is new before they arrive.
The form records the time a client asks for. It does not block out your calendar, so you stay in control of what is confirmed: check the request against your schedule, then reply to confirm it or suggest another time.
From request to confirmed appointment
Every request lands in your submissions with the service, date and time side by side, and you get an email for each new one. The first visit answer tells you who may need a longer slot or a few extra questions.
