Who this template is for
Course creators, community managers, SaaS beta programs, memberships, and service businesses use this template as the workhorse signup form on their site. It captures the essentials — name, email, phone, attribution source, and a terms checkbox — without making the new member fill out a 12-field intake form just to register. Each registration creates a Contact in your CRM Cubby tagged with the channel they came from (social media, friend referral, search, newsletter, or other), so you know which acquisition channels are working and can route the welcome sequence accordingly. The phone field is optional — keep it for high-touch programs, remove it for casual community signups.
How signups become members
Each registration creates a Contact in your CRM Cubby with the attribution source as a tag. Filter by source to see which channels are driving real signups vs. tire-kickers. Convert qualified registrations to Deals if you're running a paid membership pipeline, or simply tag everyone for a welcome sequence if you're running a free community. The terms-acceptance checkbox creates an audit trail — every accepted record is timestamped and tied to the version of your terms in effect that day, which matters when a member contests their account later. Attach the welcome packet, onboarding video, or community guidelines as a Note on the Contact so your CS team has the whole context one click away.
What's included
The fields a working registration form actually needs — name, contact, attribution, and consent. Rename, reorder, or remove any field in the editor.
Teams using this template
Online courses and educators
Capture student name, contact, and how they heard about the course — without a 20-question intake on day one.
Community managers
Discord, Slack, or forum signups with a terms checkbox that creates the compliance trail.
SaaS beta programs
Pre-qualify beta testers by attribution channel and use case before granting access.
Memberships and clubs
Membership applications: name, contact, terms — convert qualified ones to paid Deals in your CRM.
Service businesses and studios
Class registration for gyms, dance studios, art workshops — same template, swap the term checkbox for a waiver.
Local programs and bootcamps
Capture cohort signups with attribution so you know which marketing channels filled the room.
Customization notes
Most teams add a hidden UTM-source field so the attribution dropdown can be cross-checked against the campaign URL, an account-type select if you offer multiple tiers (Free / Pro / Lifetime), and a conditional follow-up question that only appears when the user picks a specific attribution source (e.g., "which podcast?" when they pick "Podcast"). For age-gated programs, add a date-of-birth field with validation. For service businesses, swap the terms checkbox for a participation waiver tailored to your liability requirements.
